To apply you must show a driver’s license or ID, birth certificate or social security cards for each child, and proof of all household income for the past 30 days.

Please send copies of above requested information to: Holiday Depot, P.O. Box 345, Lapeer, MI. 48446, or drop off at our storefront 110 W. Park St. Lapeer or upload your information on our website.

Before uploading social security cards, please make a copy, then black out the first 5 numbers.
